Provides clinical consultation, medication education, chronic disease state management, medication reconciliation, and adherence counseling for specialty patients; supervises Specialty Pharmacy Care Coordinators; collaborates with interdisciplinary teams to facilitate patient care; enrolls patients into medication management programs and assists with recruitment in clinic; maintains clinic services, policies, and workflows while engaging in ongoing trainings to stay current with pharmacology and REMS programs.
