Clinic Nurse RN with Geriatric Emphasis

Provide high-quality, patient-centered nursing care within an integrated ambulatory clinic, supporting a geriatric-focused population as a key member of an interdisciplinary team. Deliver skilled, comprehensive care by coordinating services across multiple settings, including transitions of care with Skilled Nursing Facilities, Assisted Living Facilities, and other long-term-care environments. Perform nursing procedures, facilitate team-based daily huddles, review and reconcile medications, and maintain accurate electronic health record documentation. Support chronic disease management, Medicare wellness workflows, and preventive care through functional assessments, fall-risk screening, cognitive screening, and advance care planning. Ensure continuity of care and efficient clinical flow consistent with Federally Qualified Health Center standards.
