Job Summary
Develop and implement comprehensive plans of care for PACE participants in coordination with the Care Manager and interdisciplinary team. Conduct initial nursing assessments for new enrollees, perform semi-annual re-assessments, and execute general and skilled nursing care as assigned. Triage participants to determine care priority, assist in treatment and testing, and report condition changes to providers. Participate in nursing on-call rotation to deliver after-hours and weekend assistance. Requires Colorado RN license, CPR certification, and BSN preferred. Located in Montrose, CO.
Required Qualifications
- Graduate of a school of professional nursing
- Current Colorado Registered Nurses (R.N.) License
- Cardiopulmonary Resuscitation (CPR) certification
- Valid driver's license
- Proof of insurance
- Means of transportation
