Job Summary
Provide primary care to PACE participants through physical assessments, semi-annual reassessments, and development of individualized care plans. Evaluate physical complaints and treat frail elder participants while coordinating with the Interdisciplinary Team for discharge planning. Perform comprehensive history and physicals for new enrollees and oversee residents of VOANS Skilled Nursing Facilities by evaluating them and coordinating care with physicians. Requires Monday-Friday 8:00a-4:30p schedule with rotating on-call and intermittent local travel.
Required Qualifications
- Licensed as Registered Nurse
- Graduate of a school of professional nursing
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- Either training or related experience in the job assigned
- Monday - Friday, 8:00a - 4:30p, rotating on-call after hours
- Intermittent local travel to participants' homes
Desired Qualifications
- Two (2) years' experience as a nurse practitioner in a geriatric setting
