Clinic Nurse Navigator

Cultivate and support primary care and specialty provider co-management through timely communication, inquiry, follow-up, and integration of information into care plans regarding transitions and referrals. Serve as the contact point, advocate, and informational resource for patient families, care teams, and sub-specialists while increasing continuity of care by managing relationships with ambulatory providers and authorizations. Directly develop and enhance multidisciplinary specialty clinics to improve the clinical experience for patients and families, while communicating information and participating in satellite and multidisciplinary clinics. Coordinate patient care throughout the continuum of care as a clinical resource for care management under limited direction.
