Job Summary

Lead and manage all clinical disciplines within the Center, directing day-to-day clinic operations and ensuring effective coordination across the care team. Oversee care planning, assessments, and implementation of services while identifying issues quickly to provide timely solutions. Communicate directly with staff, leadership, and external partners to maintain clinic quality, support preparation for daily meetings and family conferences, and coordinate onboarding for new enrollees. Provide training, coaching, and direction to clinical staff, delegate tasks clearly, and ensure accurate documentation in the electronic medical record. Lead quality improvement initiatives and provide RN coverage or on-call support when needed.

Required Qualifications

  • Current state license to practice nursing
  • Valid driver's license
  • One year of experience working with the frail or elderly population, or willingness to complete training
  • Experience supervising teams or leading clinical workflows
  • Strong organizational and communication skills
  • Must be able to move intermittently throughout the workday
  • Must be able to drive a motor vehicle
  • Sits, stands, bends, lifts, and moves intermittently during working hours
  • Occasional overnight or day travel for training, continuing education, business meetings, etc.
  • Is subject to lifting, carrying, and supporting Participants
  • Employment is contingent upon successful completion of required background checks, health and safety screenings, and any clearances required for healthcare programs
