Job Summary
Conduct comprehensive physical therapy assessments and treatment plans for patients with general, orthopedic, and neurologic conditions in Florida and Kentucky. Deliver creative, compassionate care that empowers patients to function independently in daily activities while ensuring they feel healthy and engaged. Maintain accurate Medicare documentation and billing/coding practices to support outcomes for residents in independent, assisted, and memory care communities. Demonstrate exceptional communication skills and coachability as a resource to patients and stakeholders.
Required Qualifications

- Working knowledge of Medicare documentation and billing/coding practices
- Graduate of an APTA or CAPTE approved physical therapy program
- A minimum of 2 years professional experience
- State Licensure
- Basic Life Support Certification via the American Heart Association
