Job Summary
Provide counseling to clients/residents in approximately 15 care facilities per month within a 60-mile radius, covering a mix of nursing, group, or long-term care settings with varying acuity levels. Daily travel is required to visit these locations. This full-time role supports a diverse team of LPNs, PMHNPs, PhDs, and LISWs within individual teams. The position offers supported autonomy with schedule flexibility, 3 weeks of accrued PTO, 10 company-paid holidays, and travel reimbursement of approximately $600/month. Candidates must hold an LISW or LISW-S credential, an Ohio license, a valid driver's license, and a reliable vehicle.
