Job Summary
Conduct initial and ongoing patient assessments, develop care plans, and manage delivery of quality pediatric home healthcare from admission through discharge. Supervise and mentor staff nurses, LVNs, and CHHAs while assigning tasks and participating in training. Maintain accurate documentation, communicate with physicians and families, and provide education on clinical processes. Ensure adherence to compliance standards, safety protocols, and quality improvement initiatives. Travel up to 33% of the time to patient homes, with physical demands including lifting up to 50 pounds. Requires current RN licensure, one year of experience, and a licensed driver's license.
Required Qualifications
- Registered nurse with current licensure to practice nursing in the state
- Must have a minimum of one year of experience as an RN
- Current CPR certification
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order
- Ability and flexibility to travel to various patient locations as needed
- Travel: up to 33% of time: Must be able to commute to patient homes in our service areas
- Sufficient physical endurance to manage tasks over extended work
- Physical Demand: Seldom (up to 33%): Standing, walking, bending, climbing stairs, reaching overhead, pushing/pulling/lifting up to 50 pounds
- Often (33-66%): Typing, finger/hand grip/grasping, driving personal vehicle
- Nearly Continuously (> 66%): Sitting, view computer screen, operate office equipment, hearing, listening, talking
- Mental Demand: Moderate to high degree of concentration due to volume, complexity, and/or 'pressure' of work
- Comprehensive understanding of professional nursing standards and practices, including compliance with Medicare Conditions of Participation and federal, state, and local regulations
- Strong observation skills and sound nursing judgment for assessing and addressing patient
- Excellent verbal and written communication skills to effectively interact with patients, families, and additional support staff
Desired Qualifications
- Bachelor's degree or higher, preferred, in nursing or other health related field
- Prior use of Electronic Medical Records (EMRs) preferred
