Job Summary
Client Support Coordinator providing psychosocial, rehabilitation, and recovery-oriented services for adults. Works with the treatment team to assess client needs, empower goal achievement, advocate across treatment, coordinate communication between clinicians, clients, and pharmacies, respond to crises, and maintain billing-ready documentation. Maintains a caseload of 20–40 clients, conducts home visits, and submits quarterly reports to the Regional Center, while ensuring compliance with agency standards and incident/mandated reporting.
Required Qualifications
- Bachelor’s degree in Social Work or related field
- Four years of varied experience as a provider of mental health services
- California driver’s license & current vehicle insurance/registration if driving
