Job Summary
Conduct thorough initial assessments and ongoing re-assessments to maintain accurate, up-to-date client care plans. Monitor client well-being, including physical, emotional, and social needs, while recommending adjustments to services or caregiving staff. Build rapport with clients and support systems through daily, weekly, and monthly proactive communications. Resolve escalations and complaints, manage caregiver introductions, and ensure compliance with state regulations and company guidelines. Perform in-person quality assurance visits covering home safety and daily routines. Support on-call and after-hours coverage as needed.
Required Qualifications
- Current drivers license and proof of insurance
Desired Qualifications
- Bachelor's Degree in Human or Health Services or related profession from an accredited university preferred
- 3 years experience in health care, elder care, social work or related industry preferred
- Licensure as an RN/LVN/MFT may be substituted for some portion of previous experience
- Excellent customer service and conflict resolution skills
- Computer proficiency and ability to document accurate and timely notes in systems related to client visits
