Client Success Associate - Remote

Manage the end-to-end referral and admission process for patients requiring private duty nursing services, ensuring timely, accurate, and compliant intake operations. Receive and coordinate new patient intakes from hospitals, physicians, and community partners, while verifying eligibility, benefits, and insurance coverage including Medicaid and commercial plans. Review referral documentation for accuracy, enter required information into the EMR, and track authorization requests and Single Case Agreements from initiation through execution. Serve as the primary point of contact for patients, families, and referral sources regarding services, rates, and admission status, maintaining ongoing communication on intake status and service readiness. Ensure compliance with agency policies, regulatory requirements, and payer guidelines while completing assigned workflow tasks and tracking metrics to support departmental performance goals.
