Job Summary
Client Care Coordinator role at Community Bridges, Inc. oversees housing coordination for individuals and families experiencing homelessness, coordinating and monitoring community-based services including behavioral health, substance use treatment, and medical care connections. Responsibilities include managing a housing caseload, coordinating care across providers, data entry for program outputs, and producing end-of-shift reports. Requires a high school diploma or GED, an associate degree in a related field preferred, 6 months recovery from substance use/mental health conditions preferred, 1-3 years of health-care related experience preferred, and Certification as a peer-support or recovery coach within 90 days of hire. 5 weeks PTO, comprehensive benefits, and competitive pay are highlighted. Required Qualifications
- Highschool diploma or GED
- Associate degree preferred in Behavioral Health
- 1-3 years of full-time Health Care related work experience preferred
- Certified Peer Support Worker or Recovery Coach certification required within 90 days of hire
- Ability to pass a criminal background check
