Client Services Associate

Coordinate and schedule referrals for new and existing home care services while providing continuity of care. Communicate pertinent client care information to caregivers and report client/caregiver issues to the departmental Director. Maintain accurate caregiver calendars and client information, and routinely interact with clients, family members, and care managers to facilitate home care services. Work in a fast-paced environment with a sense of urgency, taking independent responsibility for all referrals received. Full-Time, In Office position in Edinburg, TX with an annual salary range of $45,000.00 to $50,000.00.
