Job Summary
Provides community development guidance and case management for clients in assigned counties. Serves as a case management specialist/social worker, monitors and maintains the community service network, determines client eligibility for services, guides clients through applications and information verification, informs the public about program availability, collaborates with community organizations to provide referrals and partnerships, coordinates with field staff to improve services, documents resources and inputs data into databases, prepares purchase requisitions and voucher requests, maintains weekly expenditure logs and site documentation, collects partnership materials, manages an allocated budget for Polk County, responds to network site needs for service delivery, provides training to network sites for efficient client service, and establishes an information referral system to connect clients to community services. Performs other duties as assigned.
