Job Summary
Coordinate client intake and ongoing care for home-care clients. Responsibilities include serving as the first point of contact for inquiries, conducting in-home assessments, developing individualized care plans, and providing care coordination and ongoing support. Act as a liaison among families, caregivers, and healthcare professionals to ensure responsive, high-quality care. Responsibilities also encompass scheduling coordination, caregiver engagement, documenting in a HIPAA-compliant manner, quality assurance, client advocacy, and collaborating with the Director of Operations and care team to promote continuity of care and positive outcomes. Requires 30–35% local travel to client homes, hospitals, rehabilitation centers, and care facilities within the service area. The ideal candidate is compassionate, organized, with strong communication, problem-solving, and relationship-building skills, and capable of thriving in a fast-paced home care environment.
Required Qualifications
- Bachelor’s degree in Social Work, Nursing, Gerontology, or a related field (or equivalent experience in care coordination, case management, or healthcare)
- Minimum 3 years of experience in home care, senior services, or healthcare-related roles
- Solid knowledge of home care protocols, client assessment techniques, and care planning best practices
- Proficient in Microsoft Office (Word, Excel, Outlook) and comfortable using care systems and other health care related software
- Outstanding verbal and written communication skills; ability to communicate clearly with individuals of all backgrounds and ages
- Strong time management and organizational skills with the ability to prioritize and complete work independently
- Valid driver’s license, reliable transportation, and auto insurance
- Must be eligible to work in the U.S. and pass a criminal background check and LiveScan fingerprinting
