Job Summary

Coordinate client intake and assessments for home care clients, develop individualized care plans, and provide ongoing care coordination and support. Serve as the liaison among families, caregivers, and healthcare professionals to promote safety, independence, and quality of life. Responsibilities include guiding families through the intake process, conducting in-home assessments, scheduling, caregiver engagement, maintaining HIPAA-compliant documentation, and ensuring continuity of care. Travel locally (30–35%) to client homes, hospitals, and care facilities. Requires strong communication, organization, and problem-solving skills in a fast-paced home care environment.

Required Qualifications

  • Bachelor’s degree in Social Work, Nursing, Gerontology, or a related field (or equivalent experience in care coordination, case management, or healthcare)
  • Minimum 3 years of experience in home care, senior services, or healthcare-related roles
  • Demonstrated ability to communicate effectively with older adults, families, caregivers, and healthcare professionals
  • Valid driver’s license, reliable transportation, and auto insurance
  • Must be eligible to work lawfully in the U.S. and pass a criminal background check and LiveScan fingerprinting
