Job Summary
Oversee a team of Client Care Coordinators to deliver recovery-focused case management for clients with mental health and/or chronic health conditions. Responsible for supervising, training, and developing CCCs to address client needs, ensuring adherence to policies and funding requirements, and coordinating with external agencies. Drive quality outcomes through supervision meetings, feedback on care plans and case notes, monitoring billing and reimbursements, and maintaining compliant documentation. Requires building relationships with external organizations, field visits, and a willingness to travel within a traditional office setting. Bilingual English/Spanish a plus.
Required Qualifications
- Minimum 2-3 years of care-related experience in advocacy, mediation, and crisis management
- Bachelor's degree in Human Services, Psychology, or 5 years equivalent work experience
- 2 years prior supervisory experience highly preferred
- Computer literacy
