Job Summary
Client Care Coordinator (Night Shift) at Community Bridges, Inc. is a seasonal, shelter- or site-based role in Arizona focused on coordinating care for individuals and families experiencing homelessness. The position involves managing a housing caseload, coordinating with PCMH/PCP services, connecting clients to community-based services (including crisis, urgent care, residential treatment, medical care, medication-assisted treatment, and outpatient behavioral health), and documenting interactions and program outputs. The role requires collaboration within a multi-disciplinary team, adherence to applicable policies and procedures, data entry, shift reporting, and a commitment to addressing social determinants of health related to housing and community integration. Required Qualifications
- Highschool diploma or GED is required.
- Associate degree (or higher) in a field related to Behavioral Health is preferred.
- Minimum of 12 months of recovery from substance use and/or mental health disorders required.
- Minimum of six (6) months of paid related work experience where prior exposure to behavioral and medical crisis situations is required.
- 1-3 years of full-time Health Care related work experience where prior exposure to behavioral and medical crisis situations is preferred.
- Arizona Fingerprint Clearance Card (Program Specific).
- CBI Peer Certification within 90 days of being hired.
- Valid identification required.
