Client Care Coordinator

Coordinate in-home client visits to convert prospects and sign them up for services, serve as the primary point of contact for clients and families, conduct in-person or virtual supervisory visits, perform regular client check-ins to ensure satisfaction, support caregiver onboarding to new cases, monitor care quality and address concerns quickly, coach caregivers on client care tactics, partner with scheduling to ensure strong caregiver-client matches, document all client interactions in our management tool, participate in monthly on-call rotation, and perform field work with clients across DuPage, Cook, and Will counties.
