Job Summary
Client Care Coordinator responsibilities include delivering individual and group counseling to clients and families to support healthy relationships and emotional well-being; counseling on abusive behavior, family dynamics, substance abuse, and health/trauma-related issues; maintain client confidentiality and complete intake applications and screenings; conduct risk assessments and prepare initial biopsychosocial evaluations within 30 days; assist in developing Independent Living Plans with clients and case managers; connect clients to community resources and coordinate referrals; advocate on behalf of clients with medical, psychiatric, and educational tests; participate in case conferences to ensure collaborative service delivery; lead group counseling and psychoeducation; monitor progress via regular face-to-face contacts and document progress notes within 48 hours; ensure compliance with privacy and security policies; must obtain LMSW within one year of hire and meet pre-employment requirements including SCR/SEL clearances, fingerprinting, TB test, and CPR/First Aid and overdose prevention certifications.
