Client Care Coordinator - Day Shift

Job Summary
Coordinate care for individuals and families experiencing homelessness by managing housing caseloads and facilitating delivery of behavioral health, medical, and crisis services. Monitor and track required interactions according to Arizona Administrative Code and agency policies while completing data entry into established systems. Assist leadership with ongoing reports and document end-of-shift coordination details. Utilize housing search software to help clients identify units once matched with housing. Serve as a nexus for shelter residents by coordinating with community providers, stakeholders, and the multi-disciplinary team including nurses and counselors.
Required Qualifications
- Highschool diploma or GED
- Associate degree (or higher) in a field related to Behavioral Health
- Minimum of 12 months of recovery from substance use and/or mental health disorders
- Minimum of six (6) months of paid related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
- Valid identification
- Will complete CBI Peer Certification within 90 days of being hired
- Arizona Fingerprint Clearance Card (Program Specific)
- Seasonal role, active during peak heat periods from May to October
Desired Qualifications
- 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
