Client Care Coordinator

Job Summary
Coordinate care for individuals and families experiencing homelessness by managing a housing caseload and supporting them in ending their current episode of homelessness. Ensure coordination of care with existing Primary Care Medical Homes or Primary Care Physicians, or facilitate connection to new services for clients presenting with complex substance use, mental health, housing, and medical care needs. Work within a multi-disciplinary team including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads to serve clients with the highest acuity and greatest vulnerabilities. Complete CBI Peer Certification within 90 days of hiring and maintain valid identification and Arizona Fingerprint Clearance Card.
Required Qualifications
- Highschool diploma or GED
- Associate degree (or higher) in a field related to Behavioral Health
- Minimum of six months of recovery from substance use and/or mental health disorders
- Minimum of six (6) months of paid related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
- Valid identification
- Will complete CBI Peer Certification within 90 days of being hired
- Arizona Fingerprint Clearance Card (Program Specific)
Desired Qualifications
- 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations
