Job Summary
The Client Care Coordinator will serve individuals with mental health conditions, substance abuse issues, and/or disabilities, focusing on care coordination and client interactions across various communication channels. Responsibilities include setting client goals, monitoring progress, documenting activities, coordinating advocacy efforts, and maintaining relationships with external organizations. A Bachelor's Degree is required, with an Associate's Degree accepted if accompanied by relevant experience. Candidates must possess a valid driver's license and demonstrate computer literacy.
Required Qualifications
- Proven ability to empathize with the clients we serve
- Tenacity and passion for this work with the ability to balance objectivity with empathy
- Computer literacy required
- Valid Driver License required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed
