Job Summary
Coordinate client onboarding, caregiver placement, scheduling support, and ongoing service management. Serve as a primary operational liaison between clients, families, caregivers, internal teams, and payer requirements. Ensure accurate collection and entry of client information, monitor client satisfaction, and maintain documentation, authorizations, and compliance. Collaborate with Intake, Scheduling, Billing, and Operations teams to ensure seamless care delivery; support care continuity after start of care.
Required Qualifications
- High school diploma or equivalent
- 1–2 years experience in home care, healthcare coordination, or client services
- Strong organizational and communication skills
- Ability to manage multiple priorities in a fast-paced environment
- Experience with AlayaCare, CalAIM, or Regional Center programs preferred
