Job Summary
Conduct high-quality consultations to understand client needs and urgency, translating insights into clear care plans that drive consult-to-start conversion. Lead successful service starts by aligning expectations, coordinating care setup, and mitigating onboarding risks while ensuring smooth onboarding experiences. Drive retention and growth through ongoing client engagement, reassessments, and care plan adjustments, partnering cross-functionally with scheduling and operations teams to ensure reliable execution. Navigate sensitive family dynamics and occasional after-hours situations with empathy and strong judgment, maintaining high standards for documentation and compliance. Manage portfolio performance using data and systems to track outcomes and maintain accurate records. This role requires ~75% travel across Chicago counties, reliable transportation, and flexibility for evenings and weekends.
Required Qualifications
- 1+ year of experience in a consultative, client-facing role
- Demonstrated ability to guide clients through decisions and drive measurable outcomes
- Strong relationship management skills
- Experience owning measurable client outcomes
- Experience leading structured conversations
- Ability to partner cross-functionally to deliver services effectively and resolve execution challenges
- Comfort working within structured systems and tools
- Operational discipline, including managing follow-up, tracking performance, and maintaining accurate records
- Ability to spend ~75% of time in the field
- Reliable transportation
- Flexibility for occasional evenings/weekends
