Job Summary
Respond to new client leads and inquiries, building rapport with prospective clients, families, and referral sources. Conduct initial assessments to understand client needs, preferences, and service expectations, then document findings and coordinate with internal team members. Perform home and facility visits to complete environmental and safety checks, identifying accessibility issues or concerns that may affect service delivery. Serve as the primary point of contact for current clients and families, maintaining regular communication regarding services, changes, and needs while resolving concerns and escalating issues as appropriate. Coordinate updates with assisted living communities, discharge planners, and caregivers to ensure clear expectations and timely information flow.

Desired Qualifications
- Previous experience in home care, senior care, healthcare, client relations, case coordination, social services, or a related field
