Job Summary
Client Advocate PCAP at Sea Mar Community Health Centers. Manages a caseload of 15-20 clients, conducts home visits ~twice monthly, performs outreach to connect clients with community resources, administers clinical assessment instruments, and coordinates scheduling with community providers. Provides housing support/case management, maintains confidentiality, and collaborates with an interdisciplinary team to support client goals and integrate clients into their communities. Requires data entry, documentation, and participation in supervision and team meetings. Bachelor’s degree required; must have a vehicle and valid driver’s license, driving record must meet insurer standards. CPR/BLS and health screenings (TB test, influenza vaccine) are required, along with ongoing health screenings and a pre-employment health assessment. Works in a fast-paced, field-based environment with emphasis on client-centered, culturally competent services.
Required Qualifications
- Bachelor's Degree required
- Must have access to a car
- Valid driver’s license
- Current auto registration
- Auto insurance with minimum liability coverage
- Must authorize Sea Mar to obtain driving record from DMV (annually)
- Must pass a TB test at time of hire
- Must obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain it
- Annual influenza vaccine required (medical/religious exemptions allowed with mask requirement during flu season)
- Must carry required health screenings and certifications as applicable
- Strong organizational and time management skills
- Excellent written and oral communication skills
- Computer proficiency (Word, Excel)
- Experience engaging with high-risk mothers and families
- Ability to conduct interviews on sensitive topics
- Commitment to client-centered, culturally aware services
