Job Summary
Client Advocate PCAP at Sea Mar Community Health Centers (FQHC) provides direct outreach, home visitation case management, and advocacy services to high-risk mothers who have used alcohol and/or drugs during pregnancy, and to their families. Responsibilities include managing a caseload of 15-20 clients, conducting home visits as clinically necessary (approximately twice per month), assisting clients in setting goals based on individual needs assessments, engaging clients with community agencies, coordinating scheduling and attending case staffing and appointments, maintaining regular contact with clients’ networks, administering confidential clinical assessments, data entry, and contributing to reports and trainings. Requires a Bachelor’s Degree, access to a car and valid driver’s license, vehicle insurance, TB test, Basic Life Support/CPR certifications within specified timelines, and compliance with vaccination requirements. Strong organizational, communication, and collaborative skills; ability to work autonomously and within teams; commitment to confidentiality and culturally competent client-centered care.
Required Qualifications
- Bachelor’s Degree required
- Must have access to a car, have a valid driver’s license and current auto registration
- Must authorize Sea Mar to obtain current driving record from the DMV
- Proof of auto insurance
- Insurance endorsements page
- minimum liability coverage as required
- TB test at time of hire and annually
- Basic Life Support CPR within 90 days of hire
- BLS CPR throughout employment
- Pre-hire health screening
- Annual influenza vaccine
- Relaxed exemptions with medical/religious exemption must wear mask during flu season
