Job Summary

Engage individuals newly referred to Community Support Services for the initial 90 days of service, monitoring and coordinating necessary evaluations, surveys, and assessments to identify needs. Coordinate treatment plan development, assist clients with benefits and housing programs, and facilitate transitions to regional team staff. Provide crisis intervention and stabilization services during regularly scheduled hours, accompany individuals to appointments, and assist in daily living skills training. Support clients in achieving ISP objectives, increasing social support networks, and developing community coping skills while incorporating trauma-informed principles. Maintain required productivity standards, document progress, and attend staff meetings. Available to work a flexible schedule; Associates Degree in Human Services required, LSW licensure preferred.

Required Qualifications

  • Minimum of Associates Degree in a Human Service field
  • A car
  • Valid Ohio driver's license
  • Insurability through Agency's insurance carrier
  • Ability to work a flexible work schedule

Desired Qualifications

  • Prior experience in case management working with individuals with severe and persistent mental illnesses
  • LSW Licensure
