Job Summary
Provide clerical and administrative support to the Social Services team by preparing, maintaining, and organizing resident records, files, and social service documentation. Assist with admission, transfer, and discharge paperwork while scheduling appointments, meetings, care conferences, and resident/family meetings. Answer telephone calls, take messages, and respond to routine inquiries, directing residents and families to appropriate staff and resources. Prepare correspondence, reports, forms, and other departmental documents, and track social service referrals and follow-up activities. Order and maintain office supplies, ensuring all confidential resident information remains compliant with HIPAA and facility policies. Support compliance with federal, state, and facility regulations while performing other duties as assigned by the Director of Social Services.
Required Qualifications
- High school diploma or equivalent
- Proficiency in Microsoft Office (Word, Excel, Outlook) and electronic medical record systems
- Ability to maintain confidentiality and handle sensitive information
- Ability to work effectively with elderly residents, families, and interdisciplinary staff

- Previous clerical, administrative, healthcare, or long-term care experience
