Job Summary
Seeking a bilingual (Spanish) RN Case Manager for hospice care, responsible for patient assessment, care planning, interaction with families, and collaboration with healthcare teams. Must have a valid nursing license and experience preferred in nursing and case management.
Required Qualifications
- Graduate of an accredited school of nursing
- Valid license as a Registered Nurse in the State of Illinois
Desired Qualifications
- Hospice experience preferred
- 1 year of nursing experience preferred
- 1 year of case management experience preferred
- Valid IL driver's license and auto insurance
