Job Summary
Registered Nurse position in a hybrid role within the Commonwealth Care Alliance’s Community Intensive Care program, focusing on care management and coordination for high-risk patients. Requires in-person visits, strong clinical experience, and collaboration with interdisciplinary teams.
Required Qualifications
- Current and active state RN License
- BLS Certification
- 5+ years’ experience as Registered Nurse in a high touch clinical environment or home care
- 2+ years caring for patients/members with complex medical, behavioral health, and social needs
Desired Qualifications
- Bachelor's Degree in nursing preferred
- Experience with electronic medical record strongly preferred (eCW)
- Experience with disability issues preferred
- Experience with Medicaid or Medicare programming and insurance products (i.e. ACO, MCO, PACE, or SCO)
- Bilingual or multi-lingual
