Chronic Condition Outreach Coordinator

Job Summary
Conduct daily patient outreach to schedule appointments and close care gaps for diabetes, hypertension, and chronic kidney disease, targeting a minimum of 30 patients per day. Process internal referrals, manage inventory of medical devices, and coordinate supplies for cooking and nutrition classes. Complete SDOH screenings, assemble education packets, and distribute programming flyers while responding to EPIC messages, calls, and emails. Prepare quantitative data for grant reporting and assist staff communication on clinical policies. Attend committees, conferences, and peer learning sessions as directed. Requires EPIC experience, two years in healthcare and customer service, and flexibility for nights during reporting season. Fluency in English and Spanish is preferred.
Required Qualifications
- EPIC experience
- Minimum of two years' experience in a healthcare setting
- Minimum of two years' experience direct customer service
- Strong office and administrative skills to include MS Office and other statistical programs
- Have reliable transportation, valid driver's license, and car insurance
- Flexibility to work nights, if needed, during reporting season and for special patient centered events and programming
Desired Qualifications
- Fluency (written and verbal) in English and Spanish preferred
