Job Summary
Chronic Care Management (CCM) Assistant – Clinical Check Specialist plays a critical role in Carda Health’s private partnership programs, ensuring privacy and accuracy of patient information, converting data into EMR systems for CCM programs, conducting comprehensive past medical history reviews to identify contraindications to exercise, supporting medication reconciliation and clinical data checks, and providing general administrative support to sustain digital care delivery. The role requires a Bachelor’s degree in Clinical Exercise Physiology / Exercise Science, at least two years in a medical office setting (preferably in cardiac/pulmonary rehab), proficiency with medical records systems, strong attention to detail, knowledge of medical terminology, and familiarity with HIPAA regulations. Bonus points include ACSM Clinical Exercise Physiologist Certification, CCRP, RHIT or similar credentials, and remote work experience. The position is a CONTRACT role based in the United States with remote work options.
Required Qualifications
- Bachelor’s degree in Clinical Exercise Physiology / Exercise Science
- Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab
- Proficiency with medical records systems and software
- Excellent attention to detail and accuracy in data entry and record management
- Knowledge of medical terminology and understanding of medical record documentation
