Job Summary
Chronic Care Management-Health Coach role focuses on building patient coaching relationships to support care plans and quality of life for patients with chronic conditions. Responsibilities include communicating abnormal findings to the cardiology care team, monitoring patient adherence to care plans, escalating changes to providers to reduce hospitalizations, assisting with enrollment processes, acting as a liaison between patients and providers, working with EMR/ CCM platforms, and staying compliant with state/federal regulations. Requires experience with cardiology patients (heart failure), RN/MA/CNA credentials, remote CCM experience, EMR proficiency (Athena, NexGen, GEMMS, eClinicalWorks), and strong communication/multi-tasking; remote, day-shift, full-time position with benefits and hourly pay in the $20-$24 range based on experience.
Required Qualifications
- High School diploma or GED
- 1 year of experience in remote chronic care management
- Experience with cardiology patients - specifically heart failure
- Registered Nurse (RN) OR Medical Assistant (MA) OR Certified Nurse Assistant (CNA)
- Experience with EMRs – specifically Athena, NexGen, GEMMS and e-clinical works
- Great communication and multi-tasking skills
