Job Summary
Connect participants, their children, and families with essential community resources including healthcare, social support, and educational opportunities. Serve as the primary point of contact to offer personalized advocacy, navigate healthcare systems and insurance processes, and develop comprehensive case plans. Educate families on available services and topics related to childbirth and parenting while building relationships with local organizations. Maintain accurate confidential records and represent the program at community meetings to strengthen partnerships. Work under the supervision of the Program Director with no supervisory responsibilities.
Required Qualifications
- Ability and willingness to work at times other than regular office hours including some evenings and weekends
- Occasionally must move/carry items weigh up to 30 pounds
Desired Qualifications
- Degree in Social Work, Human Services, Public Health, or a related field preferred
- Experience will be considered in lieu of a degree
- Previous experience in advocacy, case management, or community support roles preferred
- Strong knowledge of community resources, social services, and support programs
- Excellent communication, interpersonal, and organizational skills, with a commitment to serving diverse populations
- Ability to work independently, manage multiple cases, and solve problems effectively
- Proficiency in Microsoft Office Suite and familiarity with case management software
