Job Summary
Connect families to essential community resources, including healthcare, social support, and financial assistance. Serve as the primary point of contact for participants, offering personalized advocacy to navigate healthcare systems and bureaucratic processes. Develop comprehensive case plans, track progress, and maintain accurate confidential records of all interactions. Build relationships with local organizations and represent the Choosing Childbirth Program at community events. Provide education on childbirth, parenting, and family health topics to empower participants. Participate in team meetings to contribute to program development and continuous improvement.
Required Qualifications
- Ability and willingness to work at times other than regular office hours including some evenings and weekends
- Occasionally must move/carry items weigh up to 30 pounds
Desired Qualifications
- Degree in Social Work, Human Services, Public Health, or a related field preferred
- Experience will be considered in lieu of a degree
- Previous experience in advocacy, case management, or community support roles preferred
- Strong knowledge of community resources, social services, and support programs
- Excellent communication, interpersonal, and organizational skills, with a commitment to serving diverse populations
- Ability to work independently, manage multiple cases, and solve problems effectively
- Proficiency in Microsoft Office Suite and familiarity with case management software
