Job Summary
Provide direct care to consumers in the CHIPP Supported Living Program, including medication monitoring, emergency response, and supportive counseling. Conduct periodic safety checks, assist with daily living skills like cooking and laundry, and facilitate educational and recreational activities. Accurately document progress notes, incident reports, and communication logs. Transport consumers as needed and participate in staff meetings, trainings, and quality improvement efforts. Work rotating shifts of three days on and six days off, with weekday hours from 4:00 p.m. to 7:00 a.m. and weekend hours from 4:00 p.m. to 7:00 a.m. Maintain a valid Pennsylvania driver's license and complete at least 24 hours of training annually.
Required Qualifications
- One (1) year College Credits in Social Science
- one (1) year Human Service experience
- 21 (twenty-one) years old
- must have had license for 2 years
- valid Pennsylvania Driver's License
- reliable transportation
- Pennsylvania State Police Background Check
