Job Summary
Provide crisis stabilization, case management, behavioral interventions, caregiver support, assessments, referrals, treatment planning, documentation, and coordinate with behavioral health, education, child welfare, and juvenile justice systems. Assist families in accessing services and community resources, monitor treatment progress, and participate in discharge planning and interdisciplinary collaboration. Partner with Intervention Specialists to support youth in crisis and their families in home, school, and community settings. Requires a BSW and at least two years of experience providing behavioral health interventions or case management services to children and youth, including training or experience in crisis intervention, de-escalation, and crisis management.
