Job Summary
Provide professional case management to eligible clients, act as an advocate to help secure services, and coordinate all services needed to meet clients’ needs. The Children’s Case Manager (CCM) will work with children and adolescents, collaborate within an interdisciplinary team, and flex hours to meet client demand. Travel within the designated area is required. Requires at least 1 year of relevant experience with children with Serious Emotional Disturbance, 18+ years old, and intermediate computer skills. Education must include a Bachelor’s degree in a related field or a Master’s degree in social work, education, psychology, counseling, nursing, or closely related field.
Required Qualifications
- Bachelor's Degree in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or closely related field; or a Bachelor's in an unrelated field with at least one year of relevant human services experience; or a Master's Degree in social work, education, psychology, counseling, nursing, or closely related field from an accredited graduate school.
