Job Summary
Children’s Intensive CSS role focusing on delivering Community Support Services through comprehensive case management and mental health support. Responsibilities include assessing client needs, developing and implementing treatment plans, coordinating services with other agencies, providing counseling and advocacy to clients and families, assisting with benefits, housing, and medication access, completing documentation in EMR, attending supervisory and grant-related activities, and delivering home visits as needed. Requires maintaining proper certification for CSS and completing annual trainings, with collaboration from the CSS Director and office manager. Location: Oxford, MS.
Required Qualifications
- Certification to perform CSS
- Required annual training
- Ability to submit applications independently and timely
- Provide case management services for clients meeting criteria for Community Support Services
- Provide ongoing assessment of clients’ needs and develop treatment planning
- Coordinate case management and mental health center services
- Provide supportive counseling to clients and families as needed
- Assist CSS clients in making applications for benefits and housing
- Assist clients in securing medications and developing medication management plan
- Ensure access to mental health and other services
- Provide home visits as determined by need
- Advocacy services for CSS clients
- Family counseling and education concerning mental illness to family members
- Maintain a caseload with proper documentation and paperwork
- Attend meetings and training related to CSS
- Complete reports and progress notes in EMR
- Attend supervisory meetings
- Participate in grant activities and reporting
- Perform other duties as assigned
