Job Summary
Childrens Health Home Care Manager in a full-time role located in Malone, NY, working with youth and families to develop and maintain person-centered plans. Responsibilities include completing mandated reporting, HIPAA, NYSDOH trainings, and facilitating care coordination across medical, psychiatric, social, educational, housing, and financial services. The role involves engagement and supervision of youth in school settings, collaboration with School Based Team, after-hours supervision, maintaining confidentiality, completing intakes and notes within 24 hours, and fulfilling billing requirements. Candidates should have strong organizational skills, the ability to work independently and in a team, embrace professional boundaries, travel 50%+ weekly, and promote agency services in the community. Certifications and trainings such as CANS-NY, NYSDOH trainings, and possibly TCI and Peer Specialist within specified timeframes are encouraged. Required Qualifications
- Bachelor’s degree with a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child & family studies, sociology or speech & hearing
- NYS Teacher’s Certificate for which a bachelor’s degree is required
- NYS licensure & registration as a Registered Nurse & a bachelor’s degree AND two years’ experience in providing direct services to children and families with complex needs
- Master’s Degree in any of the above programs AND one year of relevant experience
- Ability to clear NYS Criminal Justice Background Check, OCFS, DOH, NYS staff exclusion list, and be fingerprinted for the School District; OIG and HHS-OIG clearance
- Ability to travel 50% or more per week
- Must have reliable transportation and a valid driver’s license
- Must complete mandated trainings (HIPAA, Health Home Netsmart, Trauma Training) within specified timelines
- Must obtain CANS-NY certification within first 30 days and pursue NYSDOH trainings
- Possibility to obtain Peer Specialist Certification within 6 months and TCI certification renewal every 6 months
Desired Qualifications
- Bachelor’s degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child & family studies, sociology or speech & hearing
- NYS Teacher’s Certificate (bachelor’s degree required)
- NYS licensure & registration as a Registered Nurse with a bachelor’s degree and two years’ experience

- Background clearances (NYS Criminal Justice, OCFS, DOH, NYS staff exclusion list, fingerprinting)
- Willingness to travel 50% or more per week
- Valid driver’s license, insurance, and reliable transportation

- CANS-NY Certification and related NYSDOH trainings
