Children’s Case Manager

Job Summary

Conduct individual assessments to create care plans specifying presenting problems, objectives, interventions, and desired outcomes. Assist clients with applying for and maintaining medical coverage while providing outreach, advocacy, and resource identification for internal and external supports. Deliver counseling and Functional Support Services in homes, schools, or community settings, and provide Individual Therapeutic Behavioral Services to reinforce coping strategies. Offer Family Training and Counseling to promote client resiliency and intervene during acute exacerbations to ensure safety. Collaborate with treatment teams and external providers to ensure a wrap-around family systems approach. Fulfill case management contacts monthly and meet 20% of direct service key performance indicator expectations. Serve on internal and external committees as determined by the supervisor. Work primarily community-based with office and telehealth components, requiring reliable internet, transportation, and a clean driving record.

Required Qualifications

  • Bachelor's degree preferred however an Associate's degree along with a minimum of two years of direct service in a related field may qualify
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations
  • Proficient computer skills, specifically with all Microsoft office applications
  • Ability to document effectively and efficiently in course of clinical work
  • Ability to navigate an electronic health record
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision
  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services
  • Effective communication skills, organizational skills, and ability to multi-task
  • The ability to actively engage with children and conduct interactive play within a therapeutic setting
  • Manual dexterity
  • The ability to sit for extended periods of time
  • Reliable internet for remote work
  • Reliable transportation
  • A clean driving record
  • Proof of auto insurance coverage

Desired Qualifications
