Children's Case Manager 2

Coordinate and monitor case management services for high-risk individuals, developing written care plans based on assessed strengths and needs. Provide one-on-one support to recipients while maintaining relationships with families and community providers. Advocate for access to services in the least restrictive setting and resolve obstacles to self-sufficiency. Transport clients when necessary using a personal vehicle. Maintain accurate progress notes, ensure regulatory compliance, and collect data for statistical purposes. Work within the program budget to ensure high standards of service delivery.
