Children's Care Manager

Job Summary

Conduct initial consent and enrollment meetings with children and families to confirm eligibility, then enter encounter notes and upload documentation into the electronic health record. Manage a mixed caseload of outreach and enrolled cases while providing comprehensive care management, linkages, and referrals to providers. Establish supportive relationships with families experiencing behavioral health issues, substance use, or complex trauma, utilizing a safe personal vehicle for home and community-based service delivery across Suffolk County. Work within a team structure receiving weekly supervision, with flexibility required for evenings, after school hours, and weekends to meet caregiver needs.

Required Qualifications

  • A Bachelor's degree in a human service/health discipline
  • Two years' experience working with children
  • Attention to detail to ensure accuracy in the consent and enrollment process
  • Excellent inter-personal communication including the ability to establish empathic, empowering relationships with children and families who have experienced behavioral health issues, substance use, complex trauma as well as physical conditions which impact their functioning
  • Strong collaborative communication skills with health/mental health professionals, educators, child welfare services
  • Current or previous certification in completing Child and Adolescent Needs and Strengths Comprehensive Multisystem Assessments (CANS)
  • Experience with juvenile justice system
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help
  • Sound organization skills including computer skills
  • Basic knowledge of the children's public health/mental health system, child welfare, public entitlement and social service programs
  • A reliable self-starter with the ability to work both independently and as part of an interdisciplinary team
  • Flexibility in availability to work evenings, after school, weekends
  • Strong writing and computer skills
  • Basic knowledge in computer applications such as Microsoft Word, Excel and Outlook
  • Bi-lingual, English/Spanish
  • Valid and clean NYS Driver's License

  • Stipend is awarded for assessed and verified bi-lingual proficiency
