Child HH Care Manager (25760)

Engage youth and families through outreach activities until enrollment into the Children's Health Home program, then coordinate and monitor subsequent services to meet member needs. Provide care management services to children with low, medium, and high acuities, including at least one in-person contact monthly. Gather assessments, enter documentation confirming eligibility and acuity, and work with children, youth, families, and service providers to create a comprehensive Plan of Care. Identify additional providers for Plan execution, convene interdisciplinary team meetings to review progress and needs, and document transitions between systems or higher levels of care. Use NY State and Health Home information systems to input required documentation while complying with Medicaid and program billing regulations. Complete all required trainings. Valid driver's license and automobile insured per NY State requirements are necessary. Hybrid work model with local travel and personal transportation required.
