Job Summary
Child & Family Specialist responsible for ensuring continuity of services with emphasis on five case management functions: assessment, planning, linking, monitoring and advocacy. Acts as an advocate and service broker for the child and family, coordinating with SOC partners, ISD/Schools, healthcare providers, Family Court and DHHS. Responsibilities include coordinating person-centered planning, conducting comprehensive assessments of children and families, enabling access to ABA providers, Speech and OT, facilitating interdisciplinary team evaluation, coordinating benefits (e.g., SSI, Medicaid, Medicare), and maintaining documentation per policy. Requires a Bachelor’s degree in Social Work or Nursing and Michigan licensure as an LBSW/LLLBSW/Master-level clinician or RN; 1 year of relevant experience preferred. Salary range: $58,399-$66,899.
Required Qualifications
- Bachelor's Degree in Social Work from an accredited university, or a Degree in Nursing
- Must have licensure with the State of Michigan as a Licensed Bachelors Social Worker, Limited Licensed Bachelors Social Worker Licensed Master professional in a related Mental Health discipline, or licensed RN
