Child & Family Enrichment Home Visitor

Conduct home visits and facilitate parent education activities for families with children from prenatal stages through age five. Partner with families to establish goals, coordinate developmental evaluations, and manage Individual Family Goal Planning meetings. Complete intake documentation, developmental screenings, and required paperwork while utilizing trauma-informed practices. Connect families with community resources and supportive services, and maintain accurate caseload documentation. Collaborate with internal teams and community agencies to ensure positive outcomes. This role supports the organization's mission to serve vulnerable, at-risk youth and their families in Clermont County.
