Child Care Coordinator

Care Coordination role focused on NYC Medicaid beneficiaries with chronic health/behavioral health needs. Responsibilities include managing a caseload of 30–35 members, maintaining regular contact, coordinating with primary and specialty care, performing needs assessments, aiding in development and execution of care plans, assisting with access to transportation and services, facilitating referrals to community resources, supporting continuity of care, and providing crisis intervention. Requires collaboration with healthcare providers, family/caregivers, and other agencies; emphasizes health literacy-sensitive communications and documentation of progress toward measurable goals; bachelor’s degree and two years of related human service experience required.
